Personal Information and Data Protection
We like to keep all contact details as up to date as possible but under the data protection act you can advise us if you do not wish to be contacted on any of them eg. at work. The only personnel able to access your information is staff members. We will not pass your details to any outside body unless referring you to another practitioner eg. dentist, periodontist, surgeon, hospital, Primary Care Trust or Dental Business Services.
Inappropriate Violent Behaviour
Harpenden Orthodontic Practice (contractor) may notify the PCT that it will no longer provide services to a patient under the Agreement where that patient is found to have:
- Committed an act of violence against, or behaved in such a way against any person employed or visiting the above practice, and the incident has been reported to the police.
Notification may be given by any means including telephone, email, or fax but if not given in writing shall subsequently be confirmed in writing within seven days (and for this purpose a faxed or email notification is not a written one).
The time at which Harpenden Orthodontic Practice notifies the PCT shall be the time at which it makes the telephone call or sends or delivers the notification to the PCT.
The PCT shall:
- Acknowledge in writing receipt of the notice from Harpenden Orthodontic Practice and take all reasonable steps to inform the patient concerned as soon as is reasonably practicable.
